Candidates approved in the selection process for the PPG-CiAC must complete their enrollment by presenting the following documents (original and copy – front and back) at the Program Office:

Completed and signed Enrollment Form (original only);

Completed and signed Course Registration Form (original only);

Civil Identification Card showing place of birth (Driver’s License is not valid as it does not include place of birth);

Copy of CPF (Individual Taxpayer Registry) or proof of registration status, available at:

Foreigner Identity Card or Passport (identification pages – photo and signature, and page with entry stamp into the country) – For Foreigners only;

Voter Registration Card;

Military Service Certificate (when applicable);

Undergraduate Diploma or Certificate of Completion;

Master’s Degree Diploma or Certificate of Completion;

Birth or Marriage Certificate (original and copy);

A recent 3×4 cm photo
